DeafCAN! has an education, outreach, and case management contracts to serve D/HOH individuals and families in Chester County and Delaware County. We reach out and find people in need, connect them with appropriate resources and, when needed, team up with other providers to offer services. We are available to meet with individuals, families or providers related to challenges faced by or with D/HOH people.
Our Chester County contract is with the Department of Human Services and our Delaware County contract is with the Department of Behavioral Health.
Although we serve individuals and families in other counties, this is necessarily very limited without formal contract support.
